Guiding Principles of Shared Decision-Making: Health Care Team Commitment

As your health care team, we are champions of shared decision making and strive to include you in making health care decisions.

Your autonomy and right to choose medical treatments or health care screening tests, including the decision not to treat or screen is respected. We have empathy for your life story and understand how experiences, values and personal preferences impact how you make decisions. We encourage you to partner with us in the decision making process.

We regard you as a person and not just your condition. Working together as a team, we support your decisions based on goals, preferences and what matters most to you. We respect your wishes to involve your family or friends in the decision making process.
